How it works

Start from a template or a blank form. Add questions, parts and branching, then preview it before anyone else sees it.

Share
Publishing gives you a link and a QR code. A form can be open to anyone with the link, or internal to signed-in staff.

Collect
Every answer is stored here first, then copied to your team’s Google Sheet and Drive folder.
